OnTrack Diabetes is an online program for people who have Type 2 diabetes. It supports them in taking care of their health and mood, so they feel at their best. It is based on suggestions from people with diabetes and from General Practitioners, and was written by psychologists and a health care research team.
Who is OnTrack Diabetes for?
OnTrack Diabetes is for Australian adults (aged 18 – 75) with Type 2 diabetes.
How does OnTrack Diabetes work?
The program gives you information and tools on physical activity, eating, health routines, and emotions. You can do quizzes to see how their mood, diabetes care, physical activity, and eating, are going. You can set goals and make plans to keep feeling good and in control of diabetes.
How do I use OnTrack Diabetes?
The program is available 24 hours a day, at no cost, wherever an internet connection can be used.
To get the best results, we suggest that you log in at least once a week for 10 to 20 minutes and then try out the ideas from the program during the week. We give you suggestions on how to work through the program, but you can do it in any order you like, based on your own needs. You can also opt to get emails reminding you to log in if you haven't used the program for 2 weeks.
